Collection Instructions
IMPORTANT PRE-COLLECTION INFORMATION
If you are uncertain of the collection procedure after reading these instructions, please contact info@i-screen.ai. Before proceeding, please read and follow all instructions carefully. Without taking these precautions your results may be inaccurate, or may result in a possible recollection being required.
- Collect this sample on a Sunday or Monday ONLY.
- DO NOT use hair that has been dyed, bleached, permed or otherwise chemically treated. It is not acceptable for testing.
- DO NOT use underarm hair from armpits. You may use head hair, chest hair or pubic hair.
- DO NOT mix hair samples. ie. Do not mix head hair with body hair
KIT CONTENTS:
- 1 x Cardboard Scales
- 1 x Request form
- 1 x Reply Paid Post Padded Envelope
- 1 x Specimen transport bag
- 1 x Collection instructions
Specimen Collection Instructions
- Cut hair from the back of the head, at the nape of the neck. Cut hair as close to the scalp as possible.
- Cut 2.5cm to 4cm (1" - l ½") from the root end of the hair (closest to the scalp). Discard the rest of the hair. It is always recommended to provide more hair than less as this will avoid any rejections due to insufficient sample amount.
- Using the cardboard 'scales' enclosed in kit, place pencil or pen on a table underneath the card where indicated.
- Place cut hair on the 'A' half of the cardboard scales. Ensure enough hair has been placed at 'A' so scale is tipped (as shown on card) to the table top. Approximately 0.5g is needed. A visual indicator would be a heaped tablespoon.
- Fill out name, date of birth and gender on reverse of cardboard scales and place with hair into Specimen transport bag.
Sending Samples to Laboratory - Instructions
- Ensure that your i-screen request form has been correctly filled out and all patient information is correctly stated. This includes full patient name, date of birth, residential address and telephone number. Once completed, place into the unsealed section of the specimen transport bag.
- Place the specimen transport bag, which contains the i-screen pathology request form and the samples, into the padded envelope provided.
- Send your sample the same day via a Royal Mail Post Box or Post Office.
